Age-adjusted D-dimer
Age-adjusted D-dimer for patients ≥50 years (age × 10 ng/mL FEU) to rule out PE/DVT when pre-test probability is not high. ADJUST-PE formula for NCHDs.
Why use it
- Only for patients aged 50 years or older. Under 50, use the conventional laboratory cutoff (typically 500 ng/mL FEU).
- Fixed D-dimer cutoffs become less useful with age because baseline levels rise. Age adjustment increases the proportion of older patients who can safely avoid imaging when clinical probability is not high.
- Use after Wells (or equivalent) shows PE/DVT unlikely, or non-high probability. Do not use D-dimer to rule out when probability is high / “likely”.
Formula (ADJUST-PE)
- Age ≥ 50 years: cutoff = age × 10 ng/mL FEU (age × 0.01 mg/L).
- Example: age 72 → cutoff 720 ng/mL (0.72 mg/L). A result of 650 ng/mL is below cutoff (negative for rule-out).
- Negative means below the cutoff. At or above the cutoff is positive and imaging is usually needed.
- Age under 50: do not age-adjust; use the conventional cutoff (typically 500 ng/mL / 0.50 mg/L FEU).
How to use
- Confirm the patient is 50 years or older.
- Assess pre-test probability (Wells PE or DVT, or local score).
- If unlikely / non-high: send highly sensitive D-dimer.
- Apply age-adjusted cutoff (calculator above). If negative → VTE excluded without imaging in validated pathways.
- If positive → arrange CTPA (PE) or proximal leg vein ultrasound (DVT) per local protocol.
- If haemodynamically unstable with suspected PE: escalate; do not rely on D-dimer.
Important caveats
- Do not use age-adjusted cutoffs in patients under 50.
- Confirm your laboratory’s units and whether the assay reports FEU or DDU. This tool assumes FEU with a conventional 500 ng/mL cutoff.
- DDU assays often use ~250 ng/mL as the fixed cutoff and age × 5 if age-adjusted; do not mix formulas across assay types.
- Pregnancy, active cancer, recent surgery, and some local policies may modify or exclude D-dimer rule-out. Follow local VTE pathway.
- Anticoagulation before sampling can lower D-dimer and undermine rule-out.

Only for patients aged 50 years or older. Validated for highly sensitive FEU assays with a conventional cutoff of 500 ng/mL (0.50 mg/L). Confirm your lab’s units and assay. DDU assays use different cutoffs (often about half FEU).
Only for rule-out when clinical probability is not high / “likely”. Follow local protocol.
